Care Coordination / Case Managment

Care Coordination / Case Management services are provided to certain public program recipients. Includes assisting in locating and gaining access to:

Medical
Social
Educational
Other needed services, regardless of the funding source.
Coordinating the plan of care, monitoring services to maximize health and safety, and eligibility reassessments.
