Case Management Service

Case Management is a service to help your child and family obtain the mental health services your child may need. Some examples of the services available are:

  • Coordination of Services
  • Coordination with School Staff / Medical Staff
  • Day Treatment Services / Partial Hospitalization Services
  • Family Support, Education, and Advocacy
  • Referrals to Others Who Can Help
  • Respite Care Including Time Away from Home and/or Recreation Activities

  • Family Support Grant
  • Community Access for Disability Inclusion (CADI) Waiver Case Managment
  • Transition Planning for Adulthood

Goals

  • The goal is to manage symptoms and learn coping skills so that the child can remain in the community and participate in as many regular childhood activities and experiences as possible. This makes it possible to transition into productive adult citizens that contribute to the community.
  • The case manager will assist in deciding with the child and family the services needed and goals of the services.
  • These services are voluntary and the services require a monthly face-to-face contact.
